School required forms & fees due by July 31st to the ATHLETIC OFFICE
- MHSAA Approved Sports Physical (Form in Athletic Office) - Athletic Handbook Acknowledgement (Available mid-June) - $75 Pay to Play Fee (This is NOT a Cheer fee but a SCHOOL requirement) - Transportation form (REQUIRED each year - found on the back of the Athletic Handbook Form) |

* Reminder that 2 evaluation forms must be submitted:
- One from a teacher
- One from a coach in another sport
Athlete Etiquette
Student athlete will adhere to all athletic policies as articulated in the Student Athlete Handbook.
The following will not be tolerated and will result in dismissal from the team:
Disrespect to Teammates or Coaches, Insubordination, Academic Cheating, Profanity, Repeated Tardiness, Unapproved Absences, Lack of Participation
